Every organization has to deal with human error. This is always a problem, but even more so when trying to achieve the highest quality at the lowest cost with the shortest lead time. Mistakes are inevitable, but those mistakes don’t have to result in defects. Eliminating defects requires implementing programs to anticipate and avoid defects. Many companies implement a Statistical Process Control (SPC) program to predict the quantity of defects that might come from a process but knowing this is only part of the equation. Getting rid of defects altogether requires lean concepts that introduce a process for proactively addressing the mistakes identified by an SPC program before they become defects that reach customers.
